Assosiation Aviation Embraer EMB-120 Brasilia registration 5N-BJY charter flight from Lagos to Akure with 20 people on board took off from Lagos airport runway 18L but shortly after one from engines failed airplane lost height and control hit a wall from brick and crashed inside the territory of airport near airport's fuel depot killing 15 people.Airplane destoyed

The airplane was carrying the dead body of the former state governor, Olusegun Agagu together with some family members back to Akure for the burial

Quick summary:

4 sec after power set to take off power received a "take off flaps warning".The crew didn't surprised about this warning and continued normal for take off

The "set power" call was made by the Captain and the "power is set" call was confirmed by the First Officer as expected in normal operations.

Approximately 3 seconds after the "power is set" call, the First Officer noted that the aircraft was moving slowly.

Approximately 7 seconds after the "power is set" call, the internal Aircraft Voice warning system could be heard stating "Take off Flaps, Auto Feather"

The physical examination of the wreckage revealed that the right engine propeller was in the feather position and the engine fire handle was pulled/activated.

At 95 knts the First Officer asked if they would reject the take off and captain replied to continue

After rotation for take off the First Officer noticed that airplane was not climbing and asked from captain to take care for not stall the airplane

10 sec after rotation a stall warning was heard

31 sec after the stall warning airplane impacted the ground in a nose down
